HANAC, Inc. – originally founded by George Douris as the Hellenic American Neighborhood Action Committee – is a New York City-based multi-faceted social services organization. The organization was founded in 1972 to serve the needs of vulnerable populations throughout New York City.
The SARA 2 Program is seeking a professional, reliable, and community-oriented Part-Time Tenant Coordinator to support older adults living in our Senior Affordable Rental Apartment program.
The Tenant Coordinator works closely with the Program Director and program team to support tenants in maintaining stable housing, accessing appropriate services and benefits, and successfully aging in place. The position combines care coordination, tenant engagement, documentation, advocacy, and community-building responsibilities.
The Tenant Coordinator will assess tenants' social-service needs and assist in developing and maintaining individualized care plans. The position requires regular communication with tenants and coordination with healthcare providers, community organizations, benefit programs, family members, and other natural supports when appropriate.
Responsibilities include, but are not limited to, completing initial intakes, assessments, consent forms, updated assessments following significant medical or social changes, progress notes, daily case-assistance logs, and required monthly reporting. The Tenant Coordinator will provide referrals and follow up to ensure that tenants are successfully connected with needed services.
The Tenant Coordinator will assist tenants with benefits and entitlements, healthcare coordination, housing-related concerns, and other issues affecting their ability to remain safely and independently housed. The position may also include periodic home visits and wellness checks for tenants requiring additional support.
An important component of the role is helping maintain a welcoming and inclusive building community. The Tenant Coordinator will assist with educational programs, tenant activities, community events, celebrations, workshops, and initiatives supporting healthy aging and social connection.
The Tenant Coordinator will maintain professional communication with tenants, colleagues, building management, service providers, and community partners and will assist the Program Director with additional program responsibilities when needed.
Qualifications and Experience:
- Strong interpersonal and organizational skills.
- Knowledgeable in benefits and entitlements for low-income residents.
- Microsoft Office proficient (Excel, Word, PowerPoint)
- Bilingual ability is strongly preferred, English/Chinese
- Detail-oriented in prioritizing tasks.
- Reliable and possess exceptional problem-solving skills.
- Must be people-oriented, professional, and courteous with excellent phone etiquette.
- Bachelor’s Degree in Social Work, or related field; or two years of college and demonstrated experience working with aging populations.
- Must be flexible, detail-oriented, responsible, reliable, professional, and able to work as a team member, and contribute to a positive work environment.
